Abstract

Instructor’s Name Introduction Benign prostatic hyperplasia (BPH) arises from morphological changes due to the enlargement of the glandular and stomal tissues in the prostatic area. According to (Gabr et al., 2022), “BPE is thought to be a key mechanism in causing lower urinary tract symptoms (LUTS), which negatively impacts on quality of life in elderly men if left untreated.” BPH has always been associated with the aging American male. Several factors increase the chance of BPH, such as hormonal imbalance and genetics. This discussion will examine a case study of an American male, Joel, who is diagnosed with BPH with a prior condition of Parkinson's disease. These complications entirely affect his lower urinary tract. It will further discuss the pathophysiology, diagnosis, pharmacology, and nursing care practices as some of the interventions used in Joel’s treatment. The nurses play an essential role in assessing voiding dysfunction, urine retention, and UTIs and the role of BPH in these problems. Pathophysiology Clinicians are required to make quick interventions, especially when the condition affects the patient's quality of life or other organs surrounding it. Joe, 76, is unable to perform his daily activities due to his clinical condition. Understanding pathophysiology is necessary for making a balanced clinical decision for the patient. According to (Ng et al., 2020), nearly all men develop histological BPH by age 80. This show that BPH is likely to affect the aging male population. The pathophysiology of lower urinary tract symptoms in men is always associated with bladder obstruction due to prostate enlargement. The prostrate is located in the pelvis found below the bladder. Benign prostatic hyperplasia (BPH) is a condition characterized by the proliferation of the cellular elements in the prostrate, causing prostate gland enlargement. A prostate gland is a wall nut-shaped organ functioning in semen production. BPH affects the transitional and periurethral zones of the prostate(Foo, 2017). It is also characterized by voiding dysfunction due to bladder outlet dysfunction. The prostate enlargement narrows the urethra, the tube draining urine out of the bladder. This causes difficulty and pain in the flow of urine. This causes a bladder obstruction causing a UTI and Urinary retention. Clinical BPH can be differentiated from other common causes of male lower urinary tract symptoms. The abnormal anatomy is prostate adenomata, causing the variance in benign prostatic obstruction (BPO), a threat to the function of the kidney and the bladder hence a need for intervention (Lawrentschuk et al., 2016) The insertion diagnoses BPH of an indwelling catheter (IDC) into a patient’s bladder to ascertain how much urine is left after urination and prepare a 24-hour voiding diary. The doctor may also perform a digital rectum examination to check the prostate for enlargement. A prostate biopsy is administered to rule out other problems such as prostate cancer. Joe is 76 years old and is at risk of BPH due to his age. According to (), aging contributes to changes in cellular mitogenesis and hormonal homeostasis in the prostate gland leading to apoptosis; aging is also associated with inflammation provoking oxidative stress, creating a thriving environment for BPH. Joe was diagnosed with Parkinson’s disease four years ago, which affects urination. The disease affects bladder functions and nerves, prompting urination problems. Since Joe has Parkinson's disease, it heightens urinary problems, and Parkinson's disease could have fueled his contracting BPH. he has abdominal pain and a persistent urge to void, which is a characteristic of BPH. Enlarged prostrates obstructs the urine pipes by narrowing, causing pain during peeing and urination. Joe is experiencing abdominal pain, a symptom of BPH. The prolonged pain affects his posture, and the inexpressive face and the monotone are due to the pain he is suffering from. the unbalanced posture may also be a result of Parkison's disease. From the pathophysiological examination and diagnosis, we can deduce that Joe is suffering from BPH, and intervention needs to be undertaken due to the adversity of the situation. Pharmacology Prazosin and its constituents help treat BPH and is a quinazoline derivative that acts as a competitive alpha1-antagonist (Basquez et al., 2022). the BPH treatment doses of Prazosin are 0.5-1mg daily. This derivative is the most commonly used alpha-blocker; Prazosin relaxes the prostrate and bladder necks and enhances a smooth urine flow. It blocks the alpha-androgenic receptors in the bladder. The common problem with BPH is voiding, and frequent urination urges; prazosin solves this problem by relaxing the muscles and enhancing a smooth urine flow. This medication may prove ineffective after two years, and the doctor may recommend an increase in dose. According to (Moore et al., 2022), Selegiline, a monoamine oxidase (MAO) inhibitor, is administered to manage Parkinson's disease and treat depressive disorders in adults. Joe was diagnosed with the disease four years ago, and selegiline acts as an inhibitor. The medication helps in increasing the amount of dopamine in the brain. Parkison's disease symptoms are slow movement, tremors, and stiffness. Joe was shuffling as he walked towards the ward with a stooped posture. Sinemet CR (25mg Carbidopa/ 100mg Levodopa) TDS is a medication that helps treat Parkison's disease. The medication contains a combination of two medications; levodopa and Carbidopa.
